We want YOU to WIN the Maximized Living Makeover Challenge, and claim your trip to Disney World- on us! And it's easy to do.
After you've completed your 30 Day Makeover Challenge we want you to sit down and write an essay on:
"How the 5 Essentials have Maximized My Life!"
You can do this as an individual and if you are selected as the winner, you may bring a guest along with you or you can do this as a team of 2.
Your essay shouldn't be more than 1000 words (about 2 typewritten pages) and should be sincere, clever, touching, funny, and informative.

You can ALSO video-record your essay if you'd like and send that to us at the same address! (or you can send us the URL where you posted the video on line
and we'll check it out from there.)

We'll be judging all the entries and our panel of experts will be contacting you and letting you know if YOU WON!

Maximized Living Makeover Challenge Essay Rules
OFFICIAL RULES

1. HERE'S HOW IT WORKS: This is an Essay Contest
Entrants must submit an original essay of 1,000 or fewer words on the topic: "How the 5 Essentials have Maximized my Life."
Essays must be in writing and submitted electronically through our website, or recorded on video and made available to us online free of charge, or submitted
both in writing and on video. In your essay, tell us how you have used and integrated the "5 Essentials" of Maximized Living in your life over the course of 30 days of the Challenge, and describe how the "5 Essentials" have affected your life in a positive way.
Your essay may be selected to be featured in a national advertising and promotional campaign on the Internet and in print. Essays will be judged and there will be one "Grand Prize Winner". See Rule #3 for contest entry instructions and deadlines. See Rule #4 for judging criteria.
The contest is open to all qualifying participants 18 years of age or older who have completed the Maximized Living Makeover Challenge. All essays must be
submitted no later than July 14th, 2008.
Please read all of the contest rules and additional information provided below prior to entering.
2. ELIGIBILITY:
The contest is open to all qualifying residents of the United States and Canada who are 18 years of age or older as of the date of their entry and who have
completed the Maximized Living Makeover Challenge, unless prohibited by law. Members or employees of Maximized Living ("Sponsor") or any of its affiliates,
subsidiaries and agents, and the spouses, immediate family members, dependents, and/or other persons living in the same household, are not eligible to
participate in the Challenge Contest. All federal, state, and local laws and regulations apply. This contest and all entries are void where prohibited by law.
Only one entry may be submitted per email address in each contest category. If more than one entry is received from the same email address for a category,
only the first such entry received will be eligible for judging.
3. HERE'S HOW TO ENTER:
All participants in Maximized Living's Makeover Challenge receive printed information regarding the Challenge and the "5 Essentials of Maximized Living."
This information is also available online, at no charge, at:(www.maximizedlivingmakeover.com/contest).
Beginning at 9:00:00 am EST on September 27th, 2008, eligible U.S. and Canadian residents may visit the contest website at www.maximizedlivingmakeover.com and
follow the contest entry instructions to register and submit their essay electronically. A contest submission is defined as a completed contest registration, a 1,000 word or less essay submitted in writing via email to contests@maximizedliving3.com and/or recorded on video and posted to a free video-sharing website such as YouTube.com (the URL of the posting must be included in your submission email).
All submissions must be in English. Essays that are illegible, inaccessible or otherwise defective (for example, videos that have inaudible content or electronic files that cannot be accessed using commercially available software) may be disqualified by the judges. Submissions must be received no later than 11:59:59 pm EST on? November 1st, 2008.
4. JUDGING AND SELECTION OF WINNERS:
Entries will be judged by a panel of judges selected by Sponsor and comprised of Sponsor employees and doctors of chiropractic familiar with Maximized
Living's Makeover Challenge. Judges will select one Grand Prize winner. Judging will be based on the following criteria: Completeness of integration of the "5 Essentials" into the life of the entrant (50%) and sincerity, cleverness, and quality of writing and/or presentation on the topic: "How the 5 Essentials Maximized My Life" (50%). The decisions of the judges with respect to all entries and their selection of contest winners shall be conclusive and final. Sponsor reserves the right to edit any winning essay for use in promotions and advertising.
5. PRIZES AND VALUES:
One (1) Grand Prize will be awarded. The winning entrant and one guest will receive a four (4) day, three (3) night vacation package, including reasonable meal allowance, transportation, transfers to and from the Orlando airport, park entry, and accommodations, at Walt Disney World Resort in Orlando, Florida. Dates of vacation package prizes will be at the Sponsor's sole discretion. Air transportation will be on an economy fare basis. Accommodation will be "mid-range" (as defined by the Sponsor at its sole discretion), meals may be pre-arranged at a fixed dollar amount, airport transfer in Orlando will be by Disney Bus service. All ancillary purchases of souvenirs, alcoholic beverages, sundries, and amenities will be at the winner(s) sole expense. All income taxes, if applicable, attributed to the final value of the prize will be paid by the winning contest entrant(s). The cash value of this prize is based upon negotiated economy round-trip airfare, hotel accommodation for three (3) nights, and reasonable meals, not to exceed $2,000.00 for the "Individual" category winner. Winner may elect to receive the above cash values in lieu of the stated prize of Disney World vacation package. All applicable taxes will be the responsibility of the winner(s) should the cash value of the prize be accepted.
Walt Disney World Resorts is not affiliated with Sponsor and Sponsor is solely responsible for any prizes offered in this contest.
Winning entrants and essays may be featured in a national publicity campaign promoting Sponsor and its programs, at Sponsor's sole discretion. By entering
this contest, all winning entrants agree to participate in and cooperate with Sponsor in such promotions. If applicable, winner will be notified prior to any
publicity campaign. There is no monetary value associated with this portion of a prize.
6. GENERAL RULES:
All essays and material submitted with any entry, including all rights in any video essay or testimonial by a contest entrant, shall become the property of
Sponsor, with all rights reserved, and will not be returned to entrants. Essays must be original works of authorship by the entrants that have not been
previously published nor won a prize or award. Modifying, enhancing or altering a third party's preexisting work does not qualify as an original work or
authorship. Essays, including any video essays or testimonial, cannot contain any unacceptable clothing or adornments, or display any commercial/corporate
advertising (including but not limited to corporate logos, brand names, slogans, political, personal and religious statements) and they must not contain
defamatory statements (including words or symbols that are widely considered offensive to individuals of a certain race, ethnicity, religion, sexual orientation, or socioeconomic group); they must not include threats to any person, place, business, or group; they must not invade privacy or other rights of any person, firm, entity, and they must not in any way violate applicable federal, state, or local laws and regulations as determined by Sponsor, in its sole discretion. Submissions may not contain lewd or indecent language, behavior, profanities or obscenities, including but not limited to nudity or pornography, and may not in any way disparage persons or organizations associated with Sponsor. Sponsor and the judges reserve the right in their sole discretion to disqualify any entry that, in their sole opinion, refers to, depicts or in any way reflects negatively upon the Sponsor, the contest or any other person or entity, or does not comply with these requirements or these Official Rules. Submissions must be suitable for presentation in a public forum, in sole determination of the judges and/or Sponsor. A video essay or testimonial submission may NOT depict any person(s) other than the entrant(s) and may contain persons considered minors by the state or province of their residence ONLY if a parent or legal guardian of that person is part of the entry submission. Entrant(s) agree that Sponsor may use their names, likenesses and recorded voices for publicity, promotional and commercial purposes including, but not limited to posting on website(s); and entrants agree that they will hereafter provide written confirmation of such authorization upon reasonable request from Sponsor. No individual under the age of 18 may participate in the contest or under the age of 13 may appear in a video essay submission UNLESS that person is a direct member (or legal dependant) of a person that entered the video essay in the contest. Sponsor is not responsible for lost, late, incomplete, invalid, un-intelligible or misdirected entries, which are disqualified. All entries will become the exclusive property of Sponsor, and none will be acknowledged or returned. Sponsor shall have the
right to reproduce, copy, broadcast, edit, adapt and publish any or all of the essay entries in any form, and may use them in any media whatsoever throughout
the world, without limitation, for any and all advertising, promotions, and other purposes of trade, without attribution or compensation to the entrant or
his/her heirs, personal representatives, successors or assigns, or any other entity. SUBMITTING A ENTRY IN THIS CONTEST CONSTITUTES ENTRANT'S IRREVOCABLE ASSIGNMENT, CONVEYANCE AND TRANFERENCE TO SPONSOR OF ANY AND ALL RIGHT, TITLE AND INTEREST, AND ALL RESIDUAL INTERESTS IN THE VIDEO INCLUDING, WITHOUT LIMITATION, ALL COPYRIGHTS AND ANY RENEWALS AND EXTENSIONS THEREOF.
Potential contest winners (or parent/legal guardian if potential winner is deemed a minor in his/her state of residence) will be required to complete an Affidavit of Eligibility certifying that the submission is his/her own original work of authorship and he/she holds all rights to the submission, that the submission has not been previously published, has not won a previous prize or award and that he/she followed the Official Rules of this contest. Additionally, any potential contest winner (or parent/legal guardian of minor(s) if potential winner is a minor) may be required to complete a Release of Liability, Prize Acceptance Form and, if legally permissible, a Publicity Rights Release. All documents must be completed and returned within fifteen (15) days of attempted delivery of same. Noncompliance within this time period or return of any prize/prize notification as undeliverable will result in disqualification of the entry and an alternate winner may be selected. Contest winners will be notified by electronic mail. No substitution or transfer of prize by winner(s) permitted. Sponsor reserves the right to a substitute prize of equal or greater value. All federal, state, and local taxes, if applicable, are the sole responsibility of winner. In no event will more than the stated number of Grand Prizes be awarded. By accepting a prize, contest winners (or parent/legal guardian if winner is a minor) agree to hold Sponsor, its respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize. Contest winners (or parent/legal guardian if winner is a minor) assume all liability for any injury or damage caused, or claimed to be caused, by participation in this contest or use or redemption of any prize. Acceptance of prize constitutes permission for the Sponsor and its agencies to use winners' names, voices and/or likenesses for purposes of advertising and trade without further compensation, including a winners' list, unless prohibited by law. Entrants (and parent/legal guardian if participant is a minor) agree to release, discharge and hold harmless Sponsor, its respective members, directors, officers, employees and assigns, harmless from and against any and all liability and damages of any kind arising out of this contest. By participating in this contest, entrants (and parent/legal guardian if participant is a minor) agree to be bound by these Official Rules and the judges' decisions. which are final. In the event there is a discrepancy or inconsistency between disclosures or other statements contained in any contest promotional materials and the terms and conditions of the Official Rules, the Official Rules shall prevail, govern and control. All material submitted becomes the sole property of Sponsor and will not be returned. Sponsor not responsible for any typographical or other error in the printing of the offer, administration of the contest or in the announcement of the prizes.
7. WINNERS: The names of contest Grand Prize winners will be available online in the Maximized Living Makeover section of the Health Site.

NOTICE TO ONLINE ENTRANTS: ANY ATTEMPT BY AN INDIVIDUAL TO DELIBERATELY DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF
THIS CONTEST IS A VIOLATION OF CRIMINAL AND CIVIL LAWS, AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES
FROM ANY SUCH INDIVIDUAL TO THE FULLEST EXTENT PERMITTED BY LAW.
Sponsor is not responsible for faulty, incorrect or mistranscribed audio transmissions, incorrect announcements of any kind, technical hardware or software failures of any kind including any injury or damage to any person's computer related to or resulting from participating in or experiencing any materials in connection with the contest, lost or unavailable network connections, or failed, incomplete, garbled or delayed computer transmission that may limit a user's ability to participate in the contest. Sponsor reserves the right to cancel or modify the contest if fraud, misconduct or technical failures destroy the integrity of the program; or if a computer virus, bug, or other technical problem corrupts the administration or security of the program as determined by Sponsor of its judges, in their sole discretion. In the event of termination of the contest, a notice will be posted online. Any damage made to the web site will be the responsibility of the authorized e-mail account holder of the e-mail address submitted at the time of entry. Proof of submitting entries will not be deemed to be proof of receipt by Sponsor. Any use of robotic, automatic, programmed or the like methods of participation will void all such submissions by such methods. Sponsor reserves the right to prohibit the participation of any individual if fraud or tampering is suspected or if the account holder fails to comply with any requirement of participation as stated herein or with any provision in these Official Rules.
